Business license of medical devices

Certificates required for the operation of medical devices (instruments, equipment, instruments, materials or other articles used solely or in combination on the human body, including the required software).

1、办理所需资料：

(1) 《医疗器械经营企业许可申请表》；

(2) 资格证明；

(3) 营业执照或企业名称预先核准证明文件复印件；

(4) 质量管理人的资格证明；

(5) 售后服务人员的资格证明。

2、办理流程：

(1) 经营企业经办人携带上述资料前往所在地设区的市级人民政府食品药品监督管理部门申请经营许可；

(2) 工作人员受理资料，并于30个工作日内进行审查，必要时组织核查；

(3) 对符合规定条件的，准予许可并发给医疗器械经营许可证；

(4) 对不符合规定条件的，不予许可并书面说明理由。

3、医疗器械经营许可证有效期限：

医疗器械经营许可证有效期为5年，一般医疗器械生产许可证过期不可以办理延续，只能重新申请《医疗器械生产许可证》核发。
